Improving Colonoscopy Prep: Achieving Better Boston Bowel Preparation Scores

Gastroenterology and Endoscopy Season 1 Episode 11

In this episode, we discuss the importance of improving colonoscopy prep and achieving better Boston bowel preparation scores. Colon cancer is the second leading cause of cancer death in the US, but it is preventable through colonoscopies. However, the success of a colonoscopy relies heavily on proper bowel preparation. Inadequate prep can decrease polyp detection rates by 27%. Patients are advised to follow a special diet and take a bowel cleansing solution prior to the procedure to ensure a clear view of the colon. The cleanliness of the colon is graded using the Boston Bowel Preparation Scale, with a minimum score of 6 recommended for optimal polyp detection. Unfortunately, the endoscopy center discussed in the episode noticed alarmingly low Boston prep scores among their patients.

Welcome to today’s podcast episode, where we delve into an incredibly important topic - Improving Colonoscopy Prep and Achieving Better Boston Bowel Preparation Scores. Colon cancer is a major health concern, standing as the second leading cause of cancer death in the United States, claiming over 50,000 lives annually. The silver lining here is that colon cancer is preventable, and colonoscopies play a crucial role in this. They allow doctors to have a clear view of the entire colon, catching precancerous polyps before they develop into cancer. However, the success of a colonoscopy is heavily dependent on proper bowel preparation.

Now, let’s talk about the importance of colon cleansing. It’s paramount for a successful colonoscopy. Any remaining fecal matter or debris can obscure the view, leading to missed lesions. Studies have pointed out that inadequate bowel preparation can decrease polyp detection rates by a whopping 27%! So, patients are advised to stick to a special diet and take a bowel cleansing solution prior to the procedure to ensure that the colon is completely empty. This meticulous preparation enables gastroenterologists to have an unobstructed view of the colon lining.

The cleanliness of the colon during a colonoscopy is graded, and one of the validated scoring systems used is the Boston Bowel Preparation Scale. It ranges from 0, indicating an unprepared colon, to 9, signifying a perfectly clean colon. A minimum Boston score of 6 is recommended for optimal polyp detection. Anything below this points towards insufficient prep, hampering the accuracy of the colonoscopy.

In our endoscopy center, we noticed that our patients’ Boston prep scores were alarmingly low, averaging around 3, with some scores dipping to as low as 1. This was a red flag, indicating that our prep protocol needed a serious overhaul.

To address this, we brought together a stellar team including our head gastroenterologist, nurses, a patient educator, and our office administrator. We dived deep into literature, drawing from our experiences to pinpoint two primary areas for improvement - enhancing patient education around the prep process and implementing a split-dose prep regimen.

Focusing first on patient prep education, we realized its crucial role in ensuring proper colonoscopy preparation. We crafted a comprehensive, easy-to-follow instruction sheet, detailing out the entire prep process. It includes a timeline starting 3-4 days before the procedure with a low-fiber, no-seeds diet, clear illustrations of what foods are acceptable, and instructions for a full liquid diet the day before the procedure. We’ve emphasized hydration, provided tips to ease hunger, and given explicit instructions on how and when to take the bowel prep solution. We ensured that our nurses and front desk staff are well-trained to go over these instructions meticulously with our patients when scheduling, and we follow up with a call the afternoon before the procedure to reinforce these instructions and clear any last-minute queries.

Next, we introduced a split-dose bowel preparation regimen. Traditionally, patients would take the entire bowel prep solution the night before their procedure. However, research has consistently shown that split-dose regimens yield better colon cleansing results. Here, patients take half of the prep solution the afternoon or evening before the procedure, and the other half on the morning of the procedure, finishing about 4 hours prior. This method ensures that the laxative effect is at its peak during the actual procedure, as opposed to taking the full dose the night before which could allow fecal matter to accumulate again.

The results of these changes have been nothing short of transformative. Our average Boston Bowel Prep Scores shot up from around 3 to an impressive 8 out of 9. This improvement in colon cleansing efficiency means more polyps detected, and potentially, more lives saved through early colorectal cancer detection. Thorough colonoscopy prep is indispensable, and as we’ve seen, significant improvements are within reach with detailed patient instruction and the adoption of split-dose protocols.

We strongly encourage all endoscopy facilities to take a closer look at their processes and consider these strategies to achieve better outcomes. Thank you for tuning in today, and remember, early detection saves lives, and proper preparation is key. Stay informed, stay healthy!
